After you register, we will contact you with information about guitar lessons. Dates for lessons are TBD.
STUDENT ONLINE APPLICATION
Students interested in learning the basics of guitar or songwriting should complete this application. Students selected to participate in the Follow Your Heart Arts Program must attend program meetings and lessons and respond in a timely manner to communication via text or email.
STUDENT HOLD HARMLESS WAIVER / PUBLICITY RELEASE
Students or their legal guardian are required to sign the Student Hold Harmless Waiver / Publicity Release prior to the start of the first lesson. This form must be notarized and brought with you to your first lesson.
STUDENT AND PARENT AGREEMENT
Students or their legal guardian are required to sign the Student and Parent Agreement prior to the start of the first lesson.
STUDENT PRE-SURVEY
Students must complete the pre-survey before beginning the Follow Your Heart Arts Program.
STUDENT POST-SURVEY
At the completion of the Follow Your Heart Arts Program, students are asked to complete the post-survey to help us make improvements for future classes.
